HELPFUL TIPS

Please note: the application will be completed on a DocuSign website address, providing the opportunity to complete and sign your application digitally from your computer, tablet or smart phone. It is required that you have a working email address.

Please note, only for Blind Services (BSBVI), you can use 555-55-5555 in the Social Security number field as a place holder and then share your real Social Security number with BSBVI staff at intake.

Please use your full name in all pertinent places.
